Guidelines for Changing Personal Information
Address changes can be made online by logging into your Self Service Account. Click on My Profile. From there, click Addresses to update your address.
All name changes must be done in writing by completing the Change of Name/Address Form (PDF); verbal changes will not be accepted. **Proper documentation (i.e. marriage certificate, divorce decree, or court document) must be included to process a name change request. Please note that a driver’s license is not a valid form of identification for a name change.
