ADA Accommodations Policy
Disability Accommodation Policy: In accordance with the Americans with Disabilities Act, the University of Holy Cross provides disability accommodations for students with identified and/or diagnosed disabilities. Students with disabilities need not inform their instructors about the nature of their disabilities, but they are responsible for contacting and providing appropriate documentation to the Disability Services Coordinator prior to receiving accommodation. The process for requesting accommodation(s) is below:
Student requesting an accommodation should provide the following (complete the following process):
- Submit appropriate documentation from your official medical provider regarding the specific disability accommodation to the Disability Services Coordinator through https://form.jotform.com/262033673640049.
- Include the name(s) of faculty that you want to be informed of your appropriate accommodations.
- Submit your accommodation request each semester by submitting a new form.
Disability Services Coordinator shall:
- Communicate with the student regarding the requested accommodation and the determination that has been made by the Accommodations Committee.
- Email the student and requested faculty regarding the approved accommodations.
NOTE: It is the student’s responsibility to meet with each faculty member to discuss how their accommodations may be met within each course.
